What We Do
Our dedicated Lead Care Managers work closely with each member to identify barriers to care and
connect them to essential resources such as:

• Primary and specialty medical care
• Behavioral health services
• Housing and social support services
• Medication management
• Transportation and food assistance

By bridging gaps in healthcare and social services, our team ensures that individuals receive
continuous and holistic support. Our approach focuses on preventive care, crisis intervention, and
long-term health stability to help members achieve their best possible health outcomes.

Who We Serve
ECM at Access TLC is designed for individuals who experience significant health challenges and
require enhanced coordination of services. This includes, but is not limited to:

• Unhoused or housing-insecure individuals
• Adults transitioning out of institutional settings
